       Scan the completed form and email it to Steve at Avalon Clearview, with the full intention of calling ahead with your credit card info, or
       Drop in to any of the studios with the completed form, with your cheque, or
       Drop in to Avalon Clearview with your completed form, and your VISA or MasterCard or debit card.
3.    Decide what you'd like to do for your accommodations; arrange that; relax.

REFUNDS are available up to one month prior to the beginning of course. 
 
AN ADMINISTRATION FEE  will be charged on cancellation refunds.  The fee is $15 if fee
was paid by cheque or cash or debit card.  The fee is 4% of the course price if payment
was by credit card.
 
COURSE CANCELLATION by the Taste of Glass Group will refund all payments without any
administration or other fees.

SAFETY FIRST - ALL APPLICANTS MUST SIGN THIS SAFETY WAIVER.
 
The course is for ADULTS only.  Safety is our first priority.  Working with glass has some inherent risks - including, but
not limited to - cuts, splinters, burns, exposure to silica dust, lead fumes, chemicals and other toxic materials.  Students
agree to listen to, and abide by, all safety precautions given by the course instructors.
 
Students must wear closed-toed shoes at all times and use safety-glass eye protection when cutting, grinding
or working with heated glass.  Safety glasses will be available for course participants, but you may be more
comfortable bringing your own.
 
By signing below, you acknowledge that you have read the above precautions, and that you will not hold the
instructors or their staff responsible for any accidents or injury, no matter how caused.
